Apply for a Library Card

In-Person

Visit the Willamina Public Library during open hours. Look for the main check out desk, where someone will gladly assist you. Make sure to bring identification and proof of current address. Children under the age of 16 need a signature and ID from a parent or guardian.

Forms of ID that are accepted

• Driver’s license with current address
• Current Student ID card
• Other government issued ID showing name and other identifying information

Items to use for proof of current address

• Mail delivered by the Post Office within one week of the date of application
• Driver’s license with current address
• Other document showing applicant’s name and address
